Health and Safety Agreement
Sunflower Nursery School strives to maintain a clean and sanitized environment, but we cannot be held responsible for any health-related triggers, including allergens, flu, or infections that may impact your child. As per the school's standard policy, children and staff exhibiting symptoms of illness (e.g., fever, vomiting, diarrhea, or contagious diseases) must stay home until they have been symptom-free for at least 24 hours or provide a doctor’s note confirming they are no longer contagious.
Immunization Requirements: All children must be up-to-date on immunizations as per WHO health standards and regulations.
Medical Authorization
In the event of an emergency, I authorize the staff of Sunflower Nursery School to seek medical attention for my child. I understand that every effort will be made to contact me.
Health Care Instructions: Please notify the school of any known allergies, special dietary needs, or major health considerations such as ailments, surgeries, or physical care requirements that may impact your child’s well-being. A customized safety plan will be developed if necessary, and our staff will be trained to recognize and respond accordingly.
